I mean, we could also assume that Richard was lying, and the belt wasn’t an important part of his respiratory system, but, instead, had an effect that would make him unable to breathe for a period of time, if removed, in order to trick him into believing that is was.
It would explain how Cale is breathing normally now, and also is the kind of lie that Richard seems to favor. Because despite everything, Richard actually likes Cale, and probably wouldn’t take a chance at actually killing him with this kind of trickery.
